McQuaid Crew Fall 2009

REGISTRATION INSTRUCTIONS

 

1.      Submit the McQuaid Jesuit High School Athletic Form (triplicate form) to the McQuaid Nurse’s Office prior to the start of practice, accessed from the McQuaid Nurses Corner web page:

2.      Access the Crew Registration Forms and required reading from the FoSC web site:

3.      Download and complete the following Crew Registration Forms: Must be received prior to or on the first day of practice. 

a.       FoSC/GWC Waiver

b.      Rowing Acknowledgment Form

c.       Season Registration & Emergency Treatment Form

d.      Swim Test Form (required in Fall 09 Crew)

e.       McQuaid Jesuit Crew Waiver 

f.        Pittsford Tanks Waiver 

g.       McQuaid Transportation Form

h.       McQuaid Travel Permit (needed for early school dismissal as required)

 

4.      Rowers and parents are required to read the following:

a.       FoSC Boathouse Rules

b.      FoSC Crew Safety Guidelines (submit with Registration Forms page 9 VIII Rower Sign Off)

c.       FoSC Regatta Rules for Participation

d.      FoSC Parent Code of Conduct

e.       McQuaid Jesuit Crew Code of Conduct

 

5.      The Fall 2009 Crew Registration Fee is:

  • Novice Rowers (new rowers) $400.
  • Varsity Rowers (returning rowers) $500.
  • Checks to be made out to, “Friends of Scholastic Crew".  Payment options are as follows:

·        ONE-TIME installment of $500 due with Registration Forms (see step 3 above) by 9/1/09

                  Or       

·        TWO installments

Ø      1st Installment of $250 due by 9/1/09 with Registration Forms (see step 3 above)

Ø      2nd Installment of $250 due by 10/1/09, mailed to FoSC address below

            DO NOT make the checks out to McQuaid or send cash.

 

6.      The Registration Forms and Payment must be received by 9/1/09 via the following:

Ø      Given to Alex Moran in a sealed envelope with Rower’s name

                  Or

Ø      Mailed to the following:

Payment must be received by 9/1/09!

For safety reasons All Registration Forms must be

turned in prior to Rowers participation in program!!!
